General Forms:

  • New Concentrator Form & Plan of Study - Fill out this form to declare Psychology as a concentration and select a track. Then, reach out to your House's Concentration Advisor to arrange a meeting and formally declare.
  • Petition Form - Use for transfer/study abroad course credit, requirement deferral, requirement substitutions, or Non-Departmental course credit for a course that is not on the list of expedited courses. This form can be e-mailed or handed physically to the Undergraduate Office and must be accompanied by a syllabus for the petitioned course.
    • Second semester seniors must submit this form no later than 4 PM the day before the course registration deadline of their final semester. For May 2025 graduates, the deadline is November 18, 2025. For February 2027 graduates, the deadline is April 14, 2026.

Course-Specific Forms:

  • Psychology 910R Application - Students planning to take PSY 910R, Supervised Research must fill this out and have their supervisors and co-supervisors sign it. The student must then submit the form to the Undergraduate Office by the designated deadline.
    • Due date to register for Spring 2026: November 18, 2025 at 4 PM
    • Due date to register for Fall 2026: April 14, 2026 at 4 PM
  • Psychology 910R Grade Recommendation Form Researchers supervising a student in PSY 910R must evaluate their student's performance, recommend a grade for the semester, and submit this form to the Undergraduate Office by the designated deadline.
    • Due date in Fall 2025: December 15, 2025 at 4 PM
    • Due date in Spring 2026: May 11, 2026 at 4 PM
  • Psychology 910R Honor Code Template - A template for the Honor Code Affirmation that students may use when submitting the final paper to the Department. (Please note that there is a separate, distinct template to use when submitting the thesis, which can be found below under Thesis Forms).
  • Psychology 985 Application Form - Students planning to take PSY 985, Junior Thesis Tutorial must fill out this form and bring it to the Undergraduate Office by the designated deadline. Students should also consult the Department's Thesis Tutorial Instructor
    • Due date to register for Spring 2026: November 18, 2025 at 4 PM
    • Due date to register for Fall 2026: April 14, 2026 at 4 PM
  • Psychology 985 Grade Recommendation Form - PSY 985 students must fill out this form, then have their supervisor sign it and recommend a grade for the semester. The student must then submit it to the Undergraduate Office along with their 985 final paper by the designated deadline.
    • Due date in Fall 2025: December 3, 2025 at 4 PM
    • Due date in Spring 2026: April 29, 2026 at 4 PM
  • Psychology 991A Grade Recommendation Form - Thesis students who have completed the first half of PSY 991 must fill out this form, attach a progress report, then have their supervisor sign it and recommend a grade for the semester. The student must then submit it to the Undergraduate Office by the designated deadline.
    • Due date in Fall 2025: December 3, 2025 at 4 PM
    • Due date in Spring 2026: April 29, 2026 at 4 PM
  • Experiential Study in Psychology Application Form - EXPSTDY 1R provides academic credit for students who are required to receive credit in order to engage with a work or internship experience. Please see the OUE's webpage on Experiential Study for more information and enrollment instructions. 

Thesis Forms:

  • Thesis Application Form
    • Due date for Class of 2027: March 13, 2026 by 4 PM
  • Thesis Prospectus Evaluation Form - Students must bring this form to their Prospectus Evaluation Meeting, have their thesis committee fill it out, and then submit the form to the Undergraduate Office right after the meeting.
    • Due date for Class of 2026: October 3, 2025 by 4 PM
    • Due date for Class of 2027: October 2, 2026 by 4 PM
  • Undergraduate Thesis Grant Application - Students must submit this form to the Undergraduate Office no later than last day of classes in the second semester of their senior year.
  • Thesis Cover Page - Templates for the thesis cover page, which differ by concentration track.
  • Honor Code Template - Template for the Honor Code Affirmation that students may use when submitting the thesis to the Department.
